A common strategy is to alternate using the Pistol and sword in order to maximize damage in a quick period of time. Tried verifying game files and restarting PC but nothings worked so far. currently wearing green cape but the extra heart isn't showing up. Also changing capes doesn't seem to do anything. Finished horde mode to get the last cape I need for the achievements, now neither achievement will pop.
